Before the incident
walking out of the west end of the shop when supervisor noticed a technician using the forklift to unload old workboxes off of a truck into the scrap steel roll off. Supervisor went over to check on what he was doing.
What happened
Supervisor went to help the mechanic push the frame box into the roll off. As the supervisor was putting the right hand on the box the box was barely sitting on the inside of the fork. The box fell between the forks and the roll off catching the supervisors thumb.
