Before the incident
[REDACTED] was working on correcting a problem we were having with lane one infeed on the B filler.
What happened
He had the infeed cover off and was running cartons through. He was shining a flashlight and watching the cartons as they were going up the chain onto the mandrels to try to diagnose the problem. A carton got stuck and when he went to grab it with his left hand his left index finger got caught between the sprocket and infeed chain causing part of his finger to be completely severed.
