On [REDACTED] I [REDACTED] got a call from HR [REDACTED] to call EMT due to [REDACTED] suffering a fall in our [REDACTED] ball court. After reviewing footage it shows [REDACTED] was going for the ball and she lost balance causing her to fall forward. At the moment she fell she fell on top of the net causing her to hit her upper body on the metal frame and concrete. [REDACTED] was transported to the hospital via ambulance.
