He was at the tasker desk getting direction from our Supervisor [REDACTED] on his next assignment after receiving his next task he got onto the forklift and proceeded to drive away [REDACTED] noticed that he was headed toward a yellow bollard and then the forklift came to a jerking stop When the forklift came to a jerking stop [REDACTED] had placed his hand between the forklift and a yellow bollard smashing his thumb
