Maintenance working on replacing the shaft of the head roller on the pearl line incline belt. [REDACTED] was on the catwalk while [REDACTED] was below setting up the scissor lift.
What happened
[REDACTED] was moving the blend tumbler to make room for the lift. [REDACTED] had the shaft in both hands and was trying to see how the shaft would fit in the shaft weighed 30-40lbs. The shaft was lying on the belt it slipped out of [REDACTED]s hands and fell 25-30 feet hitting the front of [REDACTED] 's hat left arm and right arm.
