two team members were assigned to replace the tote containing Synergex in batching. One team member was acting as the spotter for the other who was operating the forklift. The forklift operator proceeded into the narrow opening of the chemical cage to obtain the tote. While exiting the cage the top of the forklift grazed the center grate of the metal cage creating a metal burr. Simultaneously the grate was lifted due to the force of the forklift making contact.

. The team member who was spotting jumped up and tried to slap the grate back into place and caught his right index finger on the metal burr creating a deep laceration requiring five stitches
