What happened
Employee was rigging the concrete bucket while his co-worker was getting ready to spray the bucket with form oil. When co worker lost his grip on the sprayer it fell and employee swiftly reached for it. Employee 's left foot got hung up on a piece of rebar that was on the ground and he fell over on his left side but was able to hold onto the chain bridal which prevented him from falling to the ground
