15 minute safety checks. Attempting to de-escalate the patient 's behavior.
What happened
[REDACTED] was at the end of the hallway; staff was made aware she was hitting her head off of the wall and scratching her forearm with her fingernails. Staff responded. [REDACTED] then became physically aggressive. Kicking punching clawing biting spitting at one time [REDACTED] managed to grab 2 full fistfuls of my hair and ripped a good majority of it out of my head. While awaiting help this episode of abuse did continue for 15 minutes. [REDACTED] left scratches on both hands pain to both sides of my neck and injury to my scalp from forcefully removing hair.
