Before the incident
Employee was preparing to move a large tube with a crane and double leg chain. He was picking up the tube with the crane to test the balance in order to find the center of the load.
What happened
Upon setting the load down to adjust the chains one of the chains was resting inbetween a piece of wood and the tube on the edge of the wood. Worker went to adjust the chain not knowing it was pinched with the load and the chain slipped off of the wood leaving his fingers in the space where the chain was when the tube dropped back onto the wood.
