Before the incident
Employee was up on the DAF system cleaning the screen. Employee started to descend the ladder using 3 points of contact. Before last rung of the ladder was reached employees glove slipped off ladder. This resulted in employee missing the last step and losing their balance. Employee then fell into the 12 curb around the entire system. Neck head did make contact with curb which resulted in laceration. Other nearby contractors and employee on maintenance dock heard the employee in distress. Nestle employee immediately started to engage the fallen employee in conversation to gauge the extent of injury and they called first responders. Contractors immediately started to render first from their first aid kit. After first responders arrived EMS was called. Once EMS arrived onsite they took over. Fallen employee was loaded into ambulance and taken for further evaluation.
