Employee was performing regular job duties- driving a forklift and receiving material to be stored on racks. While attempting to unload a pallet he caught the rack decking and twisted it.

The employee was attempting to fix a rack that he twisted when he hit it with a fork from the forklifts while unloading a pallet. He decided to climb up to the first deck of the racking however while doing so another employee saw and was offering to give him a boost with the other employees forks up onto the decking and after doing so the employee made it onto the decking but then slipped as he was adjusting himself on the rack. When he slipped he could not catch himself and he fell onto the ground.

Employee broke his right humerus and dislocated a finger
