What happened
Employee was cleaning up an area where some old pallets and lumber was being stored. Some of the pallets had deteriorated from being in the weather and had fallen apart. During the cleanup employee had stepped on one of the broken pallets pieces which resulted in him stepping on a nail that penetrated through his work boot and into the bottom of his right foot.
