Employee heard SSL 1 tape machine making a noise so he turned the machine off and took the covers off of both chains conveyors. After tightening the right chain he started the machine up to inspect make sure it worked.

When he was watching the right chain he set his left hand on top of the left chain and caught his fingertip between the moving chain and sprocket.
