What happened
Employee was using a motorized pallet jack to change the nests out on the wheel lip welder and debris got caught under one of the wheels which prevented it from turning. When the employee went to put the pallet jack in reverse to clear the debris he ran over his left big toe. This caused his left big toenail to be pulled away from the nailbed.
