What happened
Employee was carrying an empty tote to put on a skid at load side. Employee tripped over the angle iron on the floor causing her to fall and hit her chin on the empty tote. This resulted in a cut under her chin lower right leg a stoved right pointer finger and sprained left shoulder. Employee was taken to Salem Regional Hospital by ambulance.
