Before the incident
was using the load bar which loosens the strap holding the freight on the trailer in place. The bar slipped out of the buckle which the bar turns to loosen and tighten the straps. Due to the force needed to turn the buckle when the bar slipped out [REDACTED] fell forward hitting his head on the metal bracket that holds the mudflap on the trailer.
