RN went to get pt PRN medication and upon return to pt room he was able to free three of four his restraints working to remove his fourth restraint on his LLE RN approached pt and tried to deescalate and possibly reapply restraints on his upper extremity At this point pt swung his right arm and struck this RN to theleft side of the head at the bottom of the ear RN then proceeded to grab his wrist but pt maneuvered and was able to kick RN using his right leg
