I was doing intake and rapid triaging A She wanted to be seen for lower back pain after previously being seen [REDACTED] for the same CC She was unaccompanied when checking in I had taken her vital signs while she was sitting downWhile attempting to ask pt triage questions she states I have back pain proceeded to stand up and with a close fist hit my right ear and said bitch I immediately walked away and walked inside the triage box I told the nurses and registration to call a code grey and security because I was just punched by a patientSecurity screening in the lobby was notified code grey was activated and multiple hospital security arrived immediately Charge Nurse and House Supervisor was made aware of incident She needs to be accompanied prior to being seen at this facility from now on
