Before the incident
EE did not bring the tank breather to ground level and instead left it on an above surface where it could be knocked over and fall to the ground.
What happened
EE was in the Winemakers studio washing portable tanks. EE stepped up a platform ladder to take the lid breather off the top of tank MP12 to wash it. EE set the breather on either one of the steps or the platform at the top of the ladder. EE climbed down and moved the ladder which caused the breather to fall and hit them just above the forehead.
