What happened
The employee slipped on the wet tile floor of the breakroom and fell causing a fracture to the left arm. This was from rainwater being tracked in from outside. The door that the employee entered was not a door normally accessible from the outside but another employee opened the door from the inside and propped it open which allowed employees to enter with wet feet and enter onto a tiled floor. Employees are supposed to use the employee entrance which has a rug to wipe their wet feet on.
