What happened
Mr. [REDACTED] had been conducting maintenance on a crusher when a large rock had fallen from roughly 5ft above the individual and landed on Mr. [REDACTED] 's left foot causing a contusion. Mr. [REDACTED] did seek medical attention and went to the emergency room and had X-Rays Taken. The reuslt of the X-rays shown to be there was no borken or fractured bones. At this time Mr. [REDACTED] has been instructed to work and do activity as tolerated.
