Before the incident
TM was working on a project removing relocating cross braces in the racks. TM was riding along with another employee who was operating the order picker.
What happened
TM was on the back of the picker bed holding onto the back of the bed with both hands. As operator pulled up at intersection and stopped. When operator turned right at intersection TM let go of both points of contact to picker bed. TM was then ejected from picker bed due to the turn and momentum and not holding on.
Injury or illness
TM swung to the outside of the picker holding onto just the lanyard. TM hit the back left of his head and left mid back on the side of the order picker.
