Before the incident
[REDACTED] was foaming the scalder line 2 in the picker room when chemical was splashed into his right eye. [REDACTED] states the hose slipped out of his hand and knocked his helmet and goggles off. The hose continued to spray and chemical was splashed into [REDACTED] 's right eye.
What happened
The foaming hose slipped out of my hand hit my helmet off my head as well as my goggles. Then it hit my eye.

Right eye came into contact with chemical. Pt reports 8 10 burning pain and blurred vision to right eye.
