Operator was attaching a lift cylinder to the upper arm weldment.
What happened
Operator was attaching a lift cylinder to the upper lift arm weldment. The designated lift aid was not being used. Operator was holding the cylinder in place the rod end was resting on the lower lift arm while the base end was being pinned to the upper lift arm. The rod end slipped off the lower lift arm and the cylinder fell. The operator 's hand was on the underside of the cylinder and became pinched between the lower lift arm and cylinder when the cylinder fell. Operator was wearing all PPE.
