Operator was fixing a bent block and when the block was seated by the operator the machine automatically finished the cycle pinching her finger. Light curtain did the stop the machine from cycling.

On [REDACTED] during a regular production cycle a machine actuator jammed against a misaligned block. The operator reached into the machine to correct the position of the block. As she entered the danger zone the safety light curtain correctly activated and dumped the main air supply. However the pneumatic actuator remained under pressure because it was physically pinned against the block. When the operator moved the block to unjam it the actuator—no longer held back by the obstruction—suddenly finished its motion using the trapped air. This release of energy pinched the operator 's finger against the tooling.
