Before the incident
Male pt was very combative and swinging. I was pushing on pt 's right arm so that RN could secure the straps to the bottom of the bed. She was unable to get the slip knot secured. I squatted to secure the restraint and felt a sharp shooting pain to my lower back. 6.220:Floated at [REDACTED].Assisted confused pt to bedside commode with pca assist. Pt fell sat w o warning on commode with loss of control. My rt hand was holding the back of her pants and my left arm was holding her and. [REDACTED] was guiding her right side. Another sharp pain was felt to my lower back as she sat without warning.

4.2: strain to back when holding combative pt 's arm down with two hands while other RN was working to tie restraints under bed. Then sudden sharp pain when this writer squatted to secure straps under bed when primary nurse was unable to do so 6.2: sudden sharp pain to lower back when pt collapsed - with no body control onto commode without warning

Sudden sharp shooting pain to lower back unrelieved by tylenol and ibuprofenLater that night: sharp sudden pain when trying to sit on chair - esp the right side pulling meds from lower pyxis drawers and pulling ivf from bottom back of supply cabinet weakness to right leg when walking slight limbing and favoring right leg Sudden sharp shooting pain 9 10 when going to sit or get in car

4.2: Pushing down combative male pt 's right arm while he was trying to hit punch assault staff - security was controlling pt 's left upper and lower limb - nursing staff was also holding down right lower extremity colleague and I were holding controlling right arm.Then squatting and pulling restraint strap to get it tight and secured under bed.6.2: Sudden sharp pain to lower back while guiding pt to sit on commode. However pt collapsed on commode without warning causing sharp pain to lower back. I did not lift but the weight of that female pt was about 180lbs when she collapsed
