The employee had completed unlocking containers on the forward end of the lid in [REDACTED]. After finishing this task the employee began walking along the walkway toward the landside ladder to exit the area.

As the employee approached Bay 29 their foot slipped between the walkway and the lid causing them to fall. The employee attempted to stop the fall by grabbing the chain?link railing resulting in pain and injury to the shoulder and arm.
