What happened
PER EMPLOYEE: I gave a Sublocade subcutaneous injection to patient named [REDACTED] [REDACTED] [REDACTED]. After giving the injection I attempted to cover the needle with the safety cover that comes attached to the needle. As I pushed on the safety cover with my left thumb which is normal protocol the needle and the safety cover broke off the syringe and the needle sliced my left thumb before the needle and safety cover fell to the ground. The safety cover was hard to push. I believe the needle and safety cover were defective. I immediately washed out the laceration notified my manager and then directly went to the Emergency room for treatment. Later in the day I filled out an incident report in Midas.
