Employee arrives with supervisor about [REDACTED] [REDACTED] with a complaint of lower back pain. Employee stated that he was sitting and bending forward while working on a bent switch. There was a pull in the lower back about at the tailbone area. He stated that it was an 8 10 when it occurred around 1000 and now was 4 10. He describes it as a general ache or throbbing. He stated he was glad that he was sitting when pain occurred as he got very hot due to the pain. He has stated that he would be seen to be sure. Ice and heat were offered to employee and ice was accepted. Employee placed ice pack to lower back at his waist band. Employee has completed all testing and paperwork. He has gone to be seen at Bon Secours Urgent Care about [REDACTED]. He was driving himself and is aware to return to Health Services upon his return.
