Pushing cases

EE stated he was pushing cases of product onto a conveyor belt and felt a pop in his right shoulder. After feeling the pop in his shoulder EE stated he was feeling a significant amount of pain in the affected shoulder. EE stated the incident occurred at approximately [REDACTED] [REDACTED] on [REDACTED] and he reported to the Relief Manager at approximately [REDACTED] [REDACTED]. EE reported the incident to me [REDACTED] at [REDACTED] - I walked with him to the EHS office where he was seen by First Aid team members. An ice pack was placed on the affected shoulder while EE was assessed by the First Aid team. After assessment by First Aid EE went home and stated he will be seeking treatment [REDACTED] [REDACTED].
