What happened
While tightening a fitting using two wrenches a wrench slipped off resulting in a jerk of the tubing. The transfer pump had previously been rinsed with ethanol methanol and contained residual solvent. When the tubing jerked a quantity of solution was flicked below employee 's safety glasses and splashed onto their eye.
