What happened
After loading the secondary that presses the skybolt into the part he activated the machine. When it did not cycle he put his right hand into the machine to adjust the parts when the machine cycled crushing his ring finger between the skybolt and the part. The light curtain that was supposed to prevent the machine from cycling was not functioning properly.
