Employee was being trained on the mold punch line-learning how to pick up meat molds and transfer them to metal racks for transport.

What happened

The Machine Operator at the time bypassed the two-handed safety control to attempt to force a deformed mold through the punch machine after multiple failed punches. The Operator placed one of his hands on one of the safety control buttons and asked another coworker to press the second safety control button so that the Operator could hold the troublesome metal pan with his other hand in a way that it would be punched through. The Operator 's attention was diverted and he did not notice that [REDACTED] was reaching over to help hold the pan steady so it would punch. It’s at this moment that [REDACTED]’s right index finger became pinched within the space of the deformed pan that was not sitting flush and the mold punch railing.
