Before the incident
IW was unloading panels from a carrier to be painted and in the process of moving the empty carrier and cart out of the way.
What happened
The carrier was empty and sitting on a cart. IW pushed the carrier and it slide off the cart. The carrier fell down and the top of the carrier struck IW on the top of the head.
