Before the incident
IW was using a staging jumper cable lift for moving scaffolding boards from one level to another during tank construction.
What happened
The cable on the staging jumper broke while the IW was holding onto the load consisting of three scaffold boards and some hoses. This caused the load to quickly fall to the scaffold deck and the IW as pulled downward with the load.
