Before the incident
Returning from a building power outage
What happened
Due to the sudden building power outage the machine stopped in the middle of a tool change causing an end mill to be stuck midway down ready to be changed when shutdown occurred. Main supervisor notified the cell lead to assist the operator in recovering the machine. An attempt to take the tool out of the pocket manually when the tool suddenly retracted back into the carousel catching the operator 's hand cutting two middle fingers on his left hand.
