Troubleshooting the shavings cross over auger.
What happened
During pre-shift start up the of the Planer Mill waste system the millwrights were troubleshooting the shavings cross over auger zero speed switch fault. Two team members were outside of the mill verifying rotation of the auger while the third was inside and noticed the incline waste conveyor wasn 't running. The Team [REDACTED] (Teram [REDACTED] 3) that was inside initiated the auto start sequence for the waste conveyors the conveyor still didn 't start. Team [REDACTED] 3 went to investigate the cause suspecting it was a bad drive belt. Team [REDACTED] 3 placed their hand on the belt to check tension and the conveyor started pulling the left index and middle finger around the sheave of the drive. LOTO violation
