What happened
[REDACTED] [REDACTED] (Electronic Report) I was instructing a pt to use her NEW glucometer. I opened the NEW lancets as well as the NEW test strips. When I pulled the lid off of the lancing device to insert the lancet there was one already in the housing. - I scratched my right thumb with the lancet. I asked pt where that came from since her boxes were unopened she states her neighbor tried to show her and he used one out of his pack. So it was a used lancet.
