The maintenance team was setting up the company truck with a snow plow attachment. The injured employee exited from the employee entrance and noticed the maintenance team working on this and offered to help.

The injured employee was standing in front of the truck with a maintenance employee while a second maintenance employee was in the driver 's seat. When the driver put the truck into drive and took their foot off the break the truck suddenly lurched forward. This caused the injury employee to get knocked backwards and hit their head on the concrete ground. Days away from work still ongoing.

Serious head injury.

Possible mechanical failure with the vehicle.

Employee was assisting with installing snow plow onto the truck. While standing in front of the truck it jerked forward when put into drive. Knocked the employee backward and hitting their head on the ground.
