Moving the Poultry cruiser to the equipment elevator
What happened
While an employee was moving a poultry cruiser on the 3rd floor the employee attempted to use the equipment elevator to move the poultry cruiser to another floor. The employee opened the elevator door however the elevator was on the 1st floor. The employee drove the poultry cruiser in to the elevator shaft causing him to fall from the third floor to the top of the elevator car positioned on the first floor.
