Before the incident
Employee (handloader) was coming to assist the shipping supervisor with a stack that was coming out sideways The shipping supervisor stepped onto the conveyor to remove some of the trays from the top of the stack and place them on the floor beside the stacker The handloader stepped up onto the conveyor after the shipping supervisor to remove some additional trays from the stack as well and when he was coming down from the conveyor he didnt realize that the stack that the shipping supervisor had put down was so close to where he would need to step down So when he stepped down he stepped on the stack of trays which caused him to lose his balance and fall (less than 3 feet) and the trays that he was holding fell onto him and he hit his head on the stacker behind him
