As employee was coming back into the building slipped on ice and lost her balance and fell. She put her hands down and landed flat on her stomach and hit left side of her face on the ground. Bit lip and caused a gash on inner lip. Pain in teeth arm and shoulder as well as a bruise on abdomen.
