What happened
Injured employee was working with a second employee to stack scrap wire. The second employee was driving a fork truck with a carrier of scrap wire and was attempting to set the scrap wire down on another carrier of wire. The scrap wire on the fork truck got caught on the second carrier and pulled the carrier of scrap off the forks of the fork truck. the carrier tipped and fell striking the injured employee in the head.
