Either the grinding wheel hit into the part hard or the part came loose. This caused sparks which caught fire inside the machine. The employee discharged the fire extinguisher. The hose came off the extinguisher and discharged the Kleen agent outside the machine instead of inside. Some of the Kleen agent hit the employee in the face and past his safety glasses into his right eye.
