What happened
Employee received puncture from metal shaving shooting out from the pressure in a die press. Metal flew out of one of the holes on the press 's exterior door. The holes in the door are a part of the pressing process. The cover guarding for die press holes is not adequate and metal was able to fly out and make contact with employee 's forearm.
