Before the incident
Employee had completed welding on the west side of the bucksaw passed the torch to another employee and stepped across a hole while holding a tool.
What happened
The employee side stepped across the hole at the moveable stop by the bucksaw carrying a sledgehammer with both hands. While crossing the employee 's left foot slipped inward causing the employee to fall 2 '6 into the hole and hit their right side rib cage on the west rail as the employee fell.
