Taken the damaged core wire to the basket for damaged hoses wires in AXIS meeting area at front of plant so the leader could see the [REDACTED].
What happened
Employee saw that degate had build up. Put lock on door that controls robot and entered area to clean the degate. Degate operated by different LOTO source. When maintenance reconnected core wire the degate operated catching employees arm in between degate and frame.
