What happened
Before he could fully clear the area another crew member began pulling the chain which caused the pipe to rise. As the pipe lifted a flange on the rising pipe caught on the adjacent pipe destabilizing the setup. This caused the raised pipe to swing and make contact with the stack of three previously removed pipes. That secondary stack fell over and one of the pipes struck the worker 's left foot twisting his leg as he reflexively tried to move away.
