I was assigned to watch pt in [REDACTED] 2 for safety reasons because the pt was a fall risk Pt attempted to get out of the bed and leave so I intervened and tried to help the pt to stay in bed and pt got combative and assaulted me by punching me in my arms and when I grabbed pts arms to stop her from hitting me Pt began to assault me again by kicking and also biting me I called out for help and RN came in to assist me with pt We had to restrain the pt for about 30 mins before help arrived and during that 30 mins pt was still actively trying to kick and punch staff
