Currently complaining of mid back pain. Was seen in the emergency department where x-rays of cervical spine thoracic spine and lumbar spine were taken. Revealed no fractures. Employee denies any left shoulder pain. Denies any neurological signs such as weakness numbness or burning. Denies any head injury.
